I don’t know of any small hand-operated machines which can do the die cutting of such a form (although a hand-cranked Vandercook or other proof press would be a possibility) . If your quantities are truly small, I would make a template of lightweight metal, and use a sharp knife to cut the form.
I would think you could purchase these folders commercially, unless you need very particular paper. You could print them after cutting.
